Baptist Health Care On Demand

  • Baptist Health is is offering Care on Demand, free of charge,  with the code CARE19. Baptist Health online caregivers are prepared to follow the appropriate CDC protocols and can answer questions regarding COVID-19, evaluate a patient’s risk and provide support to relieve symptoms. Telehealth is a valuable tool in helping to prevent the spread of the virus.

Student Potential Exposure to COVID-19, Development of Potential Symptoms of COVID-19,
  and/or Testing Positive for COVID-19 Reporting Form

All FAU College of Medicine students must  complete this reporting form  immediately if they test positive for COVID-19, are exposed to COVID-19, or develop symptoms of COVID-19 which include, but are not limited to, fever, cough, shortness of breath, chills, muscle pain, new loss of taste or smell, vomiting or diarrhea, and/or sore throat. They must also notify their immediate faculty or clinical rotation supervisor/attending/preceptor, course/M3 clerkship/M4 year director, and the Deans of Student Affairs, Dr. Markowitz or Dr. Caceres, via e-mail or phone at stuartm@health.fau.edu / (561) 699-9878 or jcacere3@health.fau.edu / (561) 419-5561. In addition, they must immediately contact FAU Student Health Services (SHS) at 561-297-3512.

 

Medical Student Travel Reporting Form

Due to the pandemic, all College of Medicine students must  report any travel  outside of Florida, including international travel. This information will be sent to FAU Student Health Services to determine if the student requires self-quarantine upon returning to Florida.

 

Student COVID-19 Non-Compliance Reporting

Students must comply with COVID-19 safety measures at all times. To report non-compliance, use the forms below. Please follow the COM COVID-19 Student Non-Compliance Protocol when addressing non-compliance of safety measures.
